Athletic Participation Required Documents

The following documents need to be completed and signed for every athlete every year. Students who wish to participate at any level must complete these documents. Ascension Christian High is member of the Louisiana High School Athletic Association. This information must be completed by the deadline dates for the student to be evaluated for eligibility for the upcoming school year.
